The dividing of the adrenal vein with vessel sealing device during the laparoscopic live donor nephrectomy

Abstract
Purpose: Presenting our experience with usage of vessel sealing device in order to divide the adrenal veins during laparoscopic live donor nephrectomy.
Material and Methods: 100 patients who underwent laparoscopic live donor nephrectomy between 2010-2012 in our clinic were evaluated according to their age, duration of hospitalization, sex, side of the nephrectomy, renal and vascular anomalies, duration of operation and blood loss. In order to evaluate renal hilum vascularity, vascular reconstructed computed tomography was performed to all patients. During dividing of adrenal veins LigaSure® 5 mm ( Valleylab,Boulder, CO, USA) was used as vein sealing and closure device.
Results: Left Laparoscopic live donor nephrectomy was performed for all patients. During renal vein dissection, adrenal vein was found and divided with LigaSure® 5 mm. There were no per-operative or postoperative complications. None of the patients was converted to open surgery.
Conclusion: The use of solo LigaSure® 5 mm at dissecting and dividing adrenal vein during the laparoscopic live donor nepherctomy is a safe method. But with the feeling of safety created by these technological devices, surgeon should never compromise from the standard principals of surgery. Surgical anatomic dissection is the first and the most important step of the surgery.
