Abstract

Introduction: Thyroglossal cysts are rarely malignant change and is usually found in adults. Papillary carcinoma of thyroglossal cysts often develop. Therefore, diagnosis and treatment of thyroglossal cysts in the thyroid gland must be examined. In our case, thyroglossal cyst papillary carcinoma detected from the ground and as a result of the risk assessment made after the decision was made after thyroidectomy case are presented.
Case: Forty-six year-old male patient was 2 months was admitted with swelling on the right side of the neck. Physical examination revealed a 2x2 cm in the right lateral neck stiff, immobile mass was palpated. Cervical right supraglottic larynx tomography 3x3 cm in diameter at the level of the hyoid bone, which causes destruction of irregular, plant sizes in the millimeter revealed a mass involving calcification. Eksprolasyon patients who were operated on in the right lateral neck fixed to the hyoid bone, the hyoid and thyroid cartilage between the 4x3 cm lesion is seen on the lesion, with the right part of the hyoid bone was excised. Thyroglossal cysts and cysts developed on the basis of pathology 4x3x2.8 cm in diameter, the outer wall of the cyst invasion of thyroid papillary carcinoma was reported as not detected. Postoperative USG thyroid was normal in the thyroid gland. High-risk patients (45 years of age, tumor greater than 4 cm in diameter), including the operation was planned and underwent total thyroidectomy.
Discussion: Clinical and radiological thyroglossal cysts in the presence of normal thyroid gland in addition to total thyroidectomy in the treatment of papillary carcinoma is controversial. However, the patient risk groups according to demographic and tumor characteristics that are considered high-risk groups classified according to the treatment of patients thyroidectomy is recommended expansions.
